Through my signature below, I hereby authorize DLM - Language Lab and/or their agents, representatives, or assignees, to print my testimonial, in full or in part, and to use my photograph or video images, in any marketing materials and on their website. 

I understand my testimonial, photograph or video images will be used for informational, promotional, and instructional purposes and will not be used to generate a profit by selling them.  I understand the testimonial, photograph or video images may be used throughout the country and in print, on the Web on in broadcast publications.  I have not been compensated nor will I seek compensation for my testimonial, photograph or video images.  I hereby certify that my testimonial or endorsement of the product(s) is/are true and accurate and reflect(s) my honest belief in the quality of the product(s) that I am endorsing. I have no affiliation or connection with DLM - Language Lab, their agents, representative or assignees, or any person or entity connected with the company. I am providing this testimonial of my own free will and I have not been paid a monetary sum for doing so.  I release DLM - Language Lab and/or their agents, representatives, or assignees from responsibility should a third party violate the terms of this release.
